Round implants don't always give a fake look, but teardrop implants don't always give a natural look. For example, A high profile teardrop implant is always going to look more fake than a low profile round implant.

The greatest determinant on final outcome is pre existing breast shape, then implant style.

For those wanting extra high cleavage I like using a tall height implant so that the implant sits higher on the chest platform.

Breast Platform

If I had to give a trainee surgeon any advice is to pay close attention to the subtle differences in the breast platform pre-operatively and ensuring this is addressed in the surgical planning.

In this case the left side was 5mm lower than the right and was corrected to achieve symmetry. If you ignore these minor differences, they will create major issues post op.

Get the crease to match and everything will fall into place. Disrespect the crease at your own peril.

Inferomedial Triangle Gap

The wide inferomedial triangle gap has to be reduced to a small equilateral triangle.

This can be achieved with 3 manoeuvres:
1) Selecting wider implants
2) Over dissecting the pocket in this segment
3) Fat Grafting

Tuberosity

For patients with any degree of tuberosity to their breasts, it is essential to use fat grafting to assist the implant.

Fat helps in a number of ways:
1) Helps expand the breast platform
2) Helps minimise the risk of double bubble
3) Helps soften the cleavage
